I will comply with all relevant Cancer Research UK policies and guidelines including those outlined in the Welcome to Volunteering booklet (link is external).

To the best of my ability I will perform my role, be present at the dates and times that I have agreed, and let my agreed contact know as soon as possible if I am unable to come;

I will make sure my behaviour, attitude and language align with the aims and interests of Cancer Research UK's vision and purpose;

I will comply with health and safety guidelines, and I understand I have a responsibility for my own and others' health and safety whilst volunteering with the charity;

I agree that ownership of any intellectual property rights created in the course of my volunteering immediately vests in Cancer Research UK;

I agree that I will treat all information I have access to whilst volunteering with the charity as confidential and will not disclose any information to any other person during or after my volunteering activities without Cancer Research UK's prior consent;

I will notify Cancer Research UK of any changes in circumstances that may affect my volunteering;

I agree to provide, when asked, and if required by the role, the details of two referees who can provide a reference in support of my volunteer application;

I am volunteering entirely at my own risk and Cancer Research UK shall not be liable in any way for any injury or loss I might suffer as a result of my volunteering, unless caused by Cancer Research UK’s negligence;

I understand that if I have a disability or physical or mental health condition that might impact on my ability to volunteer, I should discuss this with my Cancer Research UK contact so that reasonable adjustments can be considered;

I agree to declare and discuss with Cancer Research UK any unspent criminal convictions I may have and am prepared to complete a DBS Check, if required by Cancer Research UK*;

I confirm that I am eligible to volunteer in the UK, and I understand that I am applying for an unpaid, voluntary role;

I declare that the answers that I have provided on this form are true and correct to the best of my knowledge. I understand that Cancer Research UK may withdraw my volunteering opportunity if it is discovered that any statement is false or misleading

I confirm that I am 18 years or older (unless Cancer Research UK has expressly stated that the age criteria for the role is less than 18 in which case I confirm that I have secured parental/guardian consent and/or supervision as necessary).
* If you are not sure whether your convictions are spent, guidance can be found here (link is external). If you have an unspent conviction, it doesn’t necessarily mean you can’t volunteer with Cancer Research UK, but we’ll need to consider it when reviewing your application.
