Job and prostate cancer

Hi I've just been accepted for a new job and my new employer has sent me a medical form to fill in ,one of the questions asks if I have cancer,I was diagnosed a year and a half ago with prostate cancer I'm on 6 monthly PSA blood test monitoring I need no treatment I'm fine but I'm worried my new employer will change his mind about me. I have to hand my notice to my present employer in two days time I really want this job but I do not want to end up unemployed.Any advice on what to do would be much appreciated.

I just wanted to quickly jump in and give you this link to information on the Macmillan website which you may find helpful.
I do know that if you have cancer the law considers this a disability and this means you cannot be treated less favourably than others who do not have cancer. If you ring the Macmillan support line tomorrow and speak to their work support team I'm sure they will be able to give you some good advice.
